A great aunt or uncle is the sibling of your grandparent. Their child is a first cousin to your parent, and a first cousin, once removed, to you. The child of your parent's first cousin is your second cousin. First cousins have the same grandparents, second cousins have the same great grandparents but not the same grandparents, and third cousins have the same great great grandparents but not the same great grandparents or grandparents and so on.

Your great grandmother's nephew is your first cousin, twice removed. In the English language/culture, his wife is not considered to be related to you and there is no specific term to describe her relationship to you as a relative. Her great uncle is certainly not related to you.

If you must refer to that person and do not care to say "the great uncle of my great grandmother's nephew's wife," the other alternative would be something like "the great uncle of the wife of my first cousin, twice removed."

The word would be sir. He is not related to you. There is no special word or phrase in English describing the father-in-law of your great uncle's cousin.
