There is one school of thought among the heavy metal subculture that the "metalhead" and "headbanger" labels represent distinctive groups within it, with metalhead being a generic term for metal fans as a whole and headbangers representing older fans or fans of older metal styles. This idea does have some merit as older heavy metal fans often dress in a distinctively 'retro' fashion from younger fans, favour different or more traditional bands and eschew some aspects of the culture that have developed in recent years. Nevertheless, both groups share common interests that go beyond a preferred musical style and together comprise a distinctive counterculture
You can say you are one if most of your music listening is metal (not goth metal, nu-metal or metalcore; those genres 'metalness' is disputed and you're liable to be called a poser if you claim to be a metalhead and then reveal your favourite band is Limp Bizkit or even HIM) and you feel metal. That sounds odd, but it's the only way I can describe it.

Anybody who simply listens to Metal or anybody from other genres of music would probably say the Metal is just music. Some arrogant idiots with no taste wouldn't even call it music, but i, being a true Metal Head through and through, and i speak here on behalf of all true Metal heads, know that Metal isn't just the most amazing and most beautiful music alive, it is with out a doubt a way of life. To belong to such a group of people, true Metal heads, is a honer to die for!

I suspect it's mainly because of their early look, with makeup and heels, but some just hate their music. As for your 2nd question, of course you are a real metal head, even if you listen to only Glam Metal (metal is in the title after all). Heavy Metal is hard to define (check out the HM doco with greats like Tony Iommi, Dio, Lemmy etc.), who all struggle to precisely define it. The best way I can define it though, is guitar orientated music, with a heavy pounded drum beat (which rules out bands like the Stone Roses), which makes you want to head bang, or at least nod your head repetitively... which is what Motley Crue's music does.
