This question is the fundamental question of the philosophical doctrine of metaphysics. Many philosophers have provided answers to this question over the last 2500 years or so, and it would be impossible to do them justice in the space available here.
Plato theorized that what was most real was the qualities of things, not their individual instantiation. Aristotle said (in his book Metaphysics which gave this branch of enquiry its name) that substance was real and the qualities were attributes which applied to the substance acccidentally.
Later, 18th century philosophers talked about whether there was a reality which we could not sense or understand which exists beyond our comprehension. Leibnitz and Spinoza said there was, empiricists like Berkeley and Hume said that some being must perceive a thing for it to be real. Immanuel Kant, in his Critique of Pure Reason, said that as human beings we are obliged to think about reality in a certain way, so that is how it must be for us.
More recently, physicists have been probing into metaphysical questions by affirming that energy and matter are fundamentally the same thing (Einstein). Physicists have clearly rejected the empiricists and generally theorize that reality is fundamentally imperceptible and can only exist for us at a conceptual level.
Well, nobody will ever know for sure until after death, but we can always speculate. One can take the Cartesian route, and say that I think, therefore I am, so we are real, and therefore what we sense is real. One could also take a Buddhist route and say the true Buddha nature is reality... There are a thousand different views of reality, you should check out a few philosophy forums and see what people are saying. The answers are surprising.
