Do not copy information verbatim from a source. Attribute all direct quotations, which you should indicate by their source in your notes.
First, use your own words, not something you found elsewhere or memorized.
Second, learn to paraphrase what others have said. That is, after reading or studying what others have written, put your thoughts together using your own writing style and vocabulary.
Third, give credit where credit is due. If you tell your reader where you found the information, then you won't be accused of plagiarizing, the original author gets credit and the reader has another source of information. Everyone wins!
The first and most obvious step would be to not look at anyone else's work. If you need to look at another source, try getting the main facts out of it and writing it down. You can then create a sentence out of the main facts.
1) Don't copy word for word, PARAPHRASEinstead.
2) Write a rough draft that is completely yours and then use resources to help you improve your writing piece (assuming that you are working on an essay)
3) Be creative! Think of your own ideas! I assure you that using your own ideas would get you a much better grade than if you copy someone else's!
Hope I helped! :)
This question seems slightly broad so I will answer questions this could mean:

First, it is important that you have strict anti-plagiarism guidelines. If this is not working there are multiple sites that will check if a piece of writing was copied from anything, the more effiencient sites may require money. If this doesn't work and a good lecture doesn't help, see a phycologist so you don't pull your hair out.
Completely and thoroughly document the sources you use in your essay
Yes, it can still be considered plagiarism if you use someone else's writing and only provide a reference without proper citation. To avoid plagiarism, you should use quotation marks for verbatim text, paraphrase in your own words, and provide a citation following the appropriate formatting style.
