B12 injections are given IM (in the muscle) . . . typically a 3cc, 23 or 25 guage x 1" needle would be used . . . Do not use an insulin syringe (too small) you may not be getting all the benefits of your treatment doing it this way.

Your doctor should be able to provide you with not only the materials to perform this task, but with the guidance necessary to perform it safetly. Failing to perform an IM injection can lead to infection and/or ulceration at the injection site. Please contact your healthcare provider.

Yes, you do need to obtain a prescription from your doctor to get Vitamin B12. He/She will also need to make sure they write you a prescription for the syringes as well. I have personally written a prescription like this one many times.
