The Law and Order series really do take stories from the headlines. When they do that, the story will start off just like the real case then take twists and turns that end the episode far differently than the real case. I know this because I'm very well versed in real crime stories and often recognize the case that the story came from, even some reasonably obscure cases. The Law and Order CI episode, 'Bombshell' is an example that most people would recognize as the case of Anna Nicole Smith. See the link below for a clip of that show.
The Lifetime channel specializes in made for TV movies about real cases. Although it's popular to satirize their programing, their movies are recreations of some very serious cases about some really bad crimes.

No Judge Judy was Not the first real judge on TV to try cases. There was the judge on the TV show; Divorce Court and the judge on the People's Court. Back in the 1950s, there were probably a lot of cheap shows, with real judges trying cases on TV, because the networks are always interested in programs that don't cost a lot of money and bring in the ratings. I doubt if Judge Judy is even the first woman judge to have her own tv show. TV has been around since the end of the 1940s. There have been a lot of shows that have aired in the last 60 years.

Depends, on the original channel 'Court TV', there were "real" court cases, such as the OJ trial. But shows like Judge Judy are more dramatized, and the cases are more often than not civil and not criminal.
