When is Medicare primary under a Group Insurance Plan? |

Answer
Medicare pays as primary when there are less than 20 employees in a group.
However, Medicare also pays as primary if you are under age 65, disabled (due to certain conditions) and covered under a group insurance plan that has less than 100 employees. http://www.medicare.gov/publications/pubs/pdf/02179.pdf
