Always go the emergency room if it is the worst headache of your life.
If it is severe and debilitating, especially if accompanied by a fever and nausea.
If you have a migraine that lasts longer than 72 hours.

You get to a hospital the same as everyone else, the best you can. Call an ambulance. If they can't get to you, they may send a helicopter to transport you.
