We celebrate the Virgin Mary's birthday on September 8, exactly nine months after the feast of the Immaculate Conception, though we are not sure of her exact birthdate. In Medjugorje, where the Virgin Mary has been appearing since 1981, she did tell the visionaries that her real birthday is August 5. So, Christians who believe in the apparitions celebrate it also on this day, but still observe the Holy Day of September 8 as well.
As for her death, we do not know exactly either, but we celebrate the Feast of the Assumption on August 15. There is controversy as to whether Mary actually died or not, but it is a church law, or dogma, that she was assumed into heaven body and soul together, and we celebrate that great event on August 15. That is why we call it the Assumption, and not an Ascension, as we describe Jesus when he left the earth for Heaven. To ascend implies that you cause it to happen yourself, you ascend. To be assumed means that it was done to you. Mary did not have the power to ascend, she was taken to Heaven by God, assumed into Heaven. Did she die first? Again, in Medjugorje, Mary has told the visionaries that she did indeed die before she was assumed into heaven. If you believe her own words at Medjugorje, then she did. Others say she was taken at the moment of death, or just before the moment of death.

There is no known date when, or if, Mary died. There are two views, both acceptable by the Catholic Church:
Mary did not die but was assumed, body and soul, into heaven.
Mary died and, within a short period (probably 3 days) her body was assumed into heaven to be reunited with her soul.
Tradition says that St. Thomas heard that Mary was near death he traveled to be with her once more but arrived three days late. When they took Thomas to the tomb, it was empty.
Some sources believe that she died in Jerusalem and others say it happened in Ephesus in modern Turkey. Both cities claim to have her tomb. Both may be correct in a way. Mary is said to have resided in Ephesus for a time and then returned to Jerusalem. She could have had a tomb constructed in Ephesus before she decided to return to Jerusalem. She would have probably left this world about the middle of the first century AD.
Clarification:
Both Jeruslaem and Ephesus claim to have the tomb of Mary but there is no evidence to support either position or whether either tomb was ever used. According to apocryphal writings, when Mary was on her death bed all the apostles were summoned. All except Thomas arrived, he was three days late. When Thomas was taken to the tomb it was found to be empty and only the burial cloths remained. The story does not give a specific location.
There are purported relics of Mary. Her veil has provided a few hairs thought to be from Mary.
