Many countries did not keep birth records for everyone in 1800. Those that did used ledger entries, not individual certificates. The form that the information was kept in was different country by country, so anything like a template would be specific to one country.
Birth certificates were not used in most of the world for most of the 1800's. Civil registration of births began in Europe during the early 1800s under Napoleon, but these were entries in bound ledgers, not certificates. Individual certificates began to be used in limited parts of the US in the very late 1800's and early 1900's, but each location used its own format.

A person will not be able to get the birth certificate from Bellevue hospital. They will need to visit the health department in that area to order a birth certificate. Hospitals do not carry them.

Go to vital records and request a copy of birth records. Ask to see Childs school file with birth record in it. If you are not on the birth certificate, you cannot request a copy.
You need to find the registrar office in the county where the birth took place.
You can find it on your birth certificate, or ask a close family member.
