Where do you find a sample medical letter addressing hardship?
There are no "samples" of such correspondence. The person should write a simple letter stating the facts of his or her circumstances and the purpose for the letter.A standard format business letter will work well for a 'medical hardship' letter:First paragraph, state why you are writing, simply, in two or three sentences.Second paragraph, state the 'facts' of your hardship (without emotion or run on thoughts); be concise. If there are many facts or pertinent information, consider using bullets. The second paragraph can be two paragraphs or more if the amount of information requires it.Third paragraph, this is where you provide contact information such as your phone number, email address, information to contact a third party (such as doctor or other related medical persons), etc. Tell the recipient what you want them to do (don't be vague or leave them guessing). If they are not required to react or respond, simply summarize from the first paragraph and thank them. The letter should be as simple as this outline.
