What kind of car? Fords up to 91-92 have it in the trunk on the drivers side over the wheel. Later models it is at the fuse panel at and above your left foot.

Pop trunk, on the left side (inside) pull back the carpet from the side, you will find a fuel pump reset switch, press it. It's not hard to find, just have to do a little diggin".

Not sure where they are on the vans, but in the cars there usally in the back, on the drive side, under the carpet on the side wall. It may be where tire and jack is on yours.
No but the pump runs only when air enters the Mass Air Flow meter. In other words the Mass Air Flow turns on the fuel pump.
