the first answer is not correct at all. Slade is Slade Wilson, aka deathstroke. He is known as Slade in Teen Titans, because of the word death in deathstroke was believed to scare off the targeted audience(pretty stupid if you ask me). His disguise in the episode 'forces of nature' is very similar to his real face. If you'd like to see his face, i believe there is an answer to the same question on yahoo answers. He is physically powered, because of military experiments that he went through. As you can tell, he is an expert in combat. His eye was destroyed by his wife, who shot him for endangering their children(which is why his mask has no right eye hole). He is very notorious in the comic book serious.
