If it's in reference to health care system, doctors, nurses and anybody else who may come in and record or refer patient to have procedures or anything else done. example: My friend was visiting her husband who had just gone thru a kidney transplant. the doctor advised the nurse to redress the wound, there was a counsellor from the organ donation group attending as well. After a few minutes my friend asked when the dressing would be redone which the nurse replied oh I'll do that later. the counsellor advised the nurse that the doctors only request of her was to redress the wound. in all it took 2 and half hours later to do it and it was done by another nurse. in all any health care worker be it doctor, nurse etc is responsible for patient care.
