George H.W. Bush (George Bush 41 ) was the last president not elected to two terms. He had low approval ratings by the end of his first term, which stuck when he was campaigning. This was mainly due to his failed promise not to increasae taxes, but he did.

FDR served for 4 terms as president, he died during his 4th term. Six years after his death congress passed the 22nd amendment that limits The President to a two term maximum.
