Thousands of Liverpool FC fans were allowed to enter an already crowded standing section of the stadium. Because of the sheer numbers that were allowed into this section, supporters found it hard to escape, hard to move and even hard to breath. As a direct result of this, 96 Liverpool supporters were either crushed to death against the fixed railings or died due to lack of oxygen (Failure to breath).

I suggest you watch some footage from that day on a popular video website to see what happened. You might have an idea now, but everything will become clear once you see it for yourself.

The Hillsborough disaster was the fault of the unruley behaviour of the Liverpool fans on the day pushing to get in - at any cost, this was compounded by (in hindsight) poor police choices as opening the gates let the crowds flood into one part of the stadium and as a result 96 people died. Though people would probably have been crushed outside had the gate not been opened.
