it depends upon the surgery. if the surgery is based on your stomach like appendicitis it would be hard for your stomach to easily digest all the foods and it might cause the stitches to break..
It is important that your stomach has no food or water before surgery because when you receive anestiesia, you might vomit and if the vomitus gets in your lungs you can die.

because if your stomach is full of food and they miss with the scapula they might gut you and food would pour out. that would be so messy.
