Before puberty both little boys and little girls look somewhat androgynous, meaning that they don't look like the sex of either adult. This is because the hormones that cause the physical transformation into man or woman have not changed them yet.

Some boys start puberty later than others, if they are a late bloomer, their body structure will appear more feminine when compared to boys of the same age who have been affected by hormones.

Also, in some boys, especially in the beginning of puberty, the large surge of hormones will cause them to temporarily develop breasts (gynecomastia). This too is normal and will go away on its own as the boy matures.
