The muscles around the break will aslo have been damaged. When the leg repairs itself it does so in a different position to its original (a pin will have insured it did not mend in a dracticaly different position, however some movement is evenvitable). The muslce are therfore having to adjust to the new bone position whilst healing and this therfore places strain on them that can cause pain and with that swelling. With time however this will decrease provided the nerves within the muscle haven't healed in posistion which can pain.
Yes. The only way to know for sure is to have it x-rayed.

If you still have pain from a place where you broke a bone after three years, the pain is not from a broken bone but from something else. Bones do not normally take that long to heal. You should find out what it is. Torn tendons can be repaired in this day and age. Bone chips can be removed. Or you can continue to put up with it for another 3 years.
Symptoms may include focused or dull/radiating pain, as well as limited mobility, intense swelling, or numbness. The best way to know if it is broken is to get x-rays from your doctor.

Well, a fragmentary pilon fracture of the distal tibia, repaired with 19 screws and two plates, hurts at about a 9. And it saps your energy for weeks.
