Factors include positional changes, exercise (which requires increased cardiac output and, thus, an increase in pressure to achieve this), arterial distensibility and disease states, among other things.

sudden repeated rapid changes in blood pressure from very high to normal may be indicative of a tumor of the adrenal glands. the adrenals produce catecholamines that elevate the blood pressure. a tumor may cause an increase in secretion of these catecholamines causing elevated blood pressure. there are cases where the tumor goes on bouts of hypersecretion and then back to normal causing rapid changes in the blood pressure. accompanying symptoms are those of the sympathetic nervous system that include sweating, increased heart rate, headache and even nausea. diagnostic testing includes a 24 hour urine sample collection that is then tested for presence of degraded catecholamines in excessive amounts. treatment generally involves excision of the tumor.

Blood pressure goes up when the body is working and exerting itself. It goes down when the body is at rest. Higher blood pressure is needed to supply blood to the body as it works. The harder the body works, the higher the blood pressure goes.
