Because people are so worn out from working so hard there whole lives. Plus, they aren't technologically advanced- they still use wax candles and horses with wagons. They society wouldn't have medicines needed to help heal the sick. And with the tight corners that everyone lives in- if one person gets sick, the disease will spread quickly.

The short life expectancy in Ayn Rand's "Anthem" is a reflection of the oppressive, collectivist society depicted in the story. The society controls every aspect of individual lives, including health care and reproduction, to maintain power and conformity. By limiting individuality and freedom, the society limits the potential for personal growth and longevity.
