first of all it depends on the type of dog you have i have a pug a nd pugs skin gets really irritated if you don't take care of it i use a special oat meal based shampoo and her skin isn't irritated... it also can be the food she eats if her food is not nutrisious enough it can be very uncomfertable..
It is possible to scratch a lot not because of the fleas but because of a food allergy or some kind of bad habit in connection with the action she/he is making immediately after the scratching like going to drink some water ,or going out..."Pavlov effect".
Dogs can also have Allergies, and dry skin. Also, how do you know there aren't any fleas? Fleas jump on a dog, bite it, then jump off. I would tell your vet next time you get your shots so he/she can check for allergies and flea dirt.
