Medal submission are made for a specific medal. As it goes through the chain of command for approvals, it is reviewed and compared to the actions of others that are being recommended for awards. They will make adjustments to the level of award to provide a consistent level of award for similar actions. I've seen a Commendation Medal reduced all the way to a Letter of Commendation.
