In most wetern countries, 16 is the minimum age for even part time employment.
In any case, yes, filling out a job application is a basic part of being interviewed, and possibly hired. It sets out your basic information as to name address and phone number as well as education level and past job record.
A propective employer has to have something in their hand to make a decision on. If they interview 10 people for one position, memory is not the way to make a proper choice.

Yes, you definately have to fill out an application -- unless you're just getting paid under the table by a family friend or something. You can usually get a worker's permit if you have valid reason to be working at age 14.
