It could be one of many things such as his bladder or urethra, a UTI, kidneys, prostate it is important he have more indepth tests done as to not worsen his condition especially since it has be going on for 3 months that is way to long and could be fatal for your puppy - he must be sad and uncomfortable you must take him to the vet or give him to someone who will take care of him. Further to the above answer which is very correct, it may be simply a behavioral problem caused by a traumatic experience or fear if the dog is routinely punished, shouted at or attacked by another pet.

Also check for any plant or home cleaning liquids that the dog may be licking. Many toxics may produce that reaction.
