For starters, she really needs to see a doctor. I would speak to my sister in a calm, non-pushing manner. Explain to her there are risks with all pregnancies. I am not sure how supportive your parents are or how they would react (i.e. if they are the supportive type, then you should try talking her into telling them, if they are not the supportive type, then she really needs to speak to a counselor (such as a prenancy hotline for teens), but many teens assume their parents want be supportive or they would flip out...in reality most parents want the best and want their children to come to them when they need help/advice, etc. This is not something that she needs to keep secret, yet I understand your dilema in wanting to be the best-friend and keep her secret. So with that I would try convincing her without arguing that she needs to tell/ seek a doctor's care and confront her parents or a counselor.
