first of all you just need to walk or drive your butt down to the hospital (with the dad of course) force the dad to sign the paper and change the last name but it would probably be easier to take his butt to court ! ;)

up until the child is one year of age it is free to change the child's name first last and middle i did! after a year there is a fee depending on where you live but all you need to do is go to your local health department and go to public records and change his name you are his mother remember to bring identification with you

If you and the mother are in conflict, it will require a court order. If you're not married to the mother, you have no legal right to the child either way. see link
