I'm pretty sure it's because of some mollusks. They find another mollusk and begin to drill through the shell until they've reached the meat within. That leaves the tiny hole. If there's multiple holes, it's because it can sometimes take a few tries to get in (because the other mollusk struggles).
Shellfish have several ocean predators, but what you are most likely seeing are holes created by the beaks of sea gulls.
